Notice of Privacy Practices

What the final notice needs to address
The approved notice should accurately describe the practice’s actual operations, including:
- how protected health information may be used and disclosed;
- the client’s privacy rights and how to exercise them;
- the practice’s legal duties and privacy responsibilities;
- how to ask questions or file a complaint, including the appropriate privacy contact;
- the notice’s effective date and how future changes will be communicated; and
- any practice-specific, California, telehealth, or other requirements that apply.
